Preschool Referral Packet for Special Needs ChildrenWelcome to the Forsyth County Exceptional Students Website dedicated to the early identification and treatment of
children with special needs. To request an evaluation for your child, you must first contact the Hill Center for an appointment to register
at 678-947-0863. Appointments may be made between 8:00 a.m. - 3:20 p.m. The following documents must be submitted at the time of your registration appointment before any evaluations
may be scheduled: l Any previous evaluations on your child l For pre-k students - Form 3300 - eye, ear, and dental. l All Screening can be obtained from either your physician or the health department at 770-781-6900 l Our county Audiologist, Teresa Gentry, is available to do hearing testing at no charge. l Call 770-887-2461 ext 312140 to schedule an appointment for a hearing test l Proof of Residency m Current lease or rental agreement and a current utility bill m Deed establishing home ownership and a current utility bill m Mortgage statement and a current utility bill m Forsyth County Schools Affidavit of Residency with notarized signatures of the landlord
and the parent.
The landlord must also provide the documents noted in one of the above bullets. l A copy of your child’s social security card l A copy of your child’s birth certificate l A copy of your child’s immunization record including the dates immunizations were administered.
