    In order for athletes on any grade level to participate in any of the summer activities, conditioning, or tryouts, they must  have a current (less than 12 months old) physical packet on file with SFHS, including many forms/components: Cover Page,  History Form, Athlete with Special Needs Form (if applicable), Physical Exam Form, Clearance Form, Concussion Awareness Form, Athletic Participation Form, and the Blanket Permission Form.
