• Health/PE Department Policies 

     

    -Students must dress out daily in tennis shoes and a West Forsyth PE Uniform.  The cost of the PE uniform is $25($10 shirt / $15 shorts).  This uniform must not be altered or damaged in any way. Failure to keep a uniform in good condition will result in the need for the student to purchase another one.

     
    -Dressing out is required. The student will receive a weekly dress out/ participation grade. Any student not dressed out for the day will not participate in the daily activity.  Improper shoes will be counted as not dressing out.
     
    -The following is a breakdown of your weekly dress out and participation grade. 5 day week/20 points per day, 4 day week/25 points per day, 3 day week/33 points per day, 2 day week/50 points per day. Each student will have a unit grade at the end of each unit that will be composed of their daily grade and their unit written test(s).
     
    -Also, grading will be based on: 80% of your grade - Formative (25%) + Summative (75%), 20% of your grade - Final exam
     
    -Students who are absent or have a doctor’s written script (note) not to participate will be required to turn in a handwritten report on the current unit of study.  A doctor’s note should include specific limitations, restrictions and length of inactivity. This must be completed and turned in to your teacher in order to receive credit.
     
    -The locker rooms will not be open until 8:20 am daily. NO exceptions. Students are given 5 minutes to dress at the beginning and end of class. NO food is allowed in the locker room. 
     
    - No students are allowed back in the locker rooms during class time. The ONLY exception would be a student with an emergency check-out. And, that student must be escorted by a coach of his/her gender to let them in the locker room.   
     
    - If a student arrives as an excused (with a pass from a faculty member) or unexcused tardy they are to go to directly to their teacher.   
     

    -Students are to use the 2 locker room facilities in the main gymnasium for water and restrooms. Only 1 student is allowed at a time, and the student must have the teacher's permission.
     

    - If a student is aware that he/she will be checking out, they are not to dress for class. (They would not be allowed back in the locker room to change when they would need to leave). 

     
    -The locker rooms will be secured and locked a 3:30pm daily. NO students or athletes will be allowed to enter until 8:20am the next school day.
     
    -It is the students’ responsibility to secure their belongings with a lock that they must rent from the department for $5.00. They will have use of the lock for the duration of their class.
     
    -Students are not allowed to leave the gym, field and/or any instructional area without the teacher’s permission.
     

    -No food, drink or gum is allowed in the gym.  Detentions will be assigned for violations of this rule.  All detentions will be served with the teacher who assigned the detention. Failure to serve will result in an office referral.

     

    - Teachers reserve the right to change any section of the course outline at any time during the course to more adequately meet the needs, abilities and interests of the students.

     

    - Students are responsible for cleaning out their locker one week prior to the semester ending. (Fall and Spring). The PE department will clean the locker rooms completely on the last day of the semester and is not liable for any lost items.
