678.965.5052 FAXIf your child is ill and diagnosed by a physician with any contagious or communicable illness please notify Nurse Surio, KSurio87@forsyth.k12.ga.us, as it is important for us to keep track of infectious illnesses in our schools.- We need to protect ourselves from exposure to all communicable diseases through prevention, hand washing, keeping hands from face, coughing into our sleeve and cleaning surfaces.
- If your child is ill please keep them at home to protect others.
