Medications At School

• A Request for Administration of Medication form must be completed by the parent or guardian for ANY medications to be given. All prescription medications or any medications that will be given longer than 2 weeks will require a physician's signature. Please see the Health Forms section of this web page to download this form.

• All medications, both prescription and over the counter, must be brought in by a parent or guardian. No students are allowed to bring medications to school, and no students may have medications on the bus. Students may not have medication in their possession except with a physician's order for emergency situations only. There is a zero tolerance for drugs and alcohol in Forsyth County Schools.

• Medications must be in their original container. Medications brought in a ziploc bag or unmarked container will not be given. Prescription medications must be in the pharmacy container labeled with the child's name, date, name of the medication, name of prescribing physician, time(s) medication is to be given and name of the pharmacy filling the prescription. We request that you ask the pharmacist to give you two labeled prescription bottles so that you have one bottle at home and one at school. For inhalers, please ask the pharmacist to apply the label around the inhaler rather than on the box of the inhaler.

• The clinic does not stock medications. All over the counter medications must be brought by a parent or guardian.

• Do not bring medications to school which needs to be given once daily or two/three times a day unless the physician specifically states a time during the school day for the medication to be given. We do not want instructional time to be interrupted unless medically necessary.

• It is the responsibility of the parent/guardian to inform the school of any changes in the child's medical treatment. Please be aware of your child's treatment plan and understand that unless communicated otherwise, current treatment will continue.
