The main goal of the Whitlow clinic is to keep all students as well and as comfortable as possible to help facilitate their full academic potential. If your child needs medication during the school day, please remember a parent/guardian needs to bring the medication to the school in its original container and sign a "Request for Administration of Medication" form, which can be found under the "Health Forms" tab to the left. Georgia Law requires a signed "Request for Administration of Medication Form" that must include a Doctors signature and parent signature to be kept on file in the clinic. The bottom portion of the form is to be completed by a physician for ANY long-term medication that will be taken for more than 2 weeks. No student will be allowed to bring medication to school or have in their possession, except with a physician's order for emergency situations only - and form signed by the physician and parent acknowledging responsibility for the medication carried. Medication brought in bags or other unmarked containers will not be given. It is the responsibility of the parent/guardian to inform the school of any changes in pertinent data, new diagnoses, and emergency contacts. Please, no cough drops or hard candies for elementary age children due to the risk of choking. Please notify the school nurse with any health concerns that may need attention while your child is attending school.
