Change of Address Information
(Currently Enrolled Students Only)
Please note that by submitting the FCS Change of Address form you are stating that you physically reside full time at the address listed on the form as of the date of submission. Changes of address cannot be post-dated.- If you own your residence, please upload the following documents to the Online Change of Address form.
- A current (within 60 days) utility (gas, electric, water, residential telephone, & cable are acceptable)* AND
- A deed, mortgage statement, property tax statement, or settlement statement (HUD).
- If you rent your residence, please upload the following documents to the Online Change of Address form.
- A current (within 60 days) utility (gas, electric, water, residential telephone, & cable are acceptable)* AND
- If your leasing agency uses Conservice for utilities, Conservice may provide you with proof of initiation of utility service.
- If utilities are included in your lease/rental agreement, the signed agreement must state that ALL utilities are included.
- A current lease/rental agreement signed by the owner of the residence.
- If you are unable to provide both proofs of residency in your name, please complete an Affidavit of Residency and upload the following document to the Online Change of Address form.
- A complete Affidavit of Residency form AND
- A current (within 60 days) utility of the owner of the residence (gas, electric, water, residential telephone, & cable are acceptable)* AND
- A deed, mortgage statement, property tax statement, or settlement statement (HUD) of the owner of the residence (OR current lease/rental agreement signed by the owner of the residence).
*If you have not received a utility bill, you may contact one of your utility companies to request proof that utility service has been initiated. Utilities must contain the account holder's name and service address. Please provide both the front and back of Sawnee EMC bills. Payment remit coupons are not acceptable.
You will be contacted by email if additional documents are needed to complete your change. You may check Parent Portal to confirm/see when your address has been updated.
Please contact your child's school or the Transportation Department after you have confirmed that your address has been updated in Parent Portal if you need assistance with transportation.

If you are unable to complete/submit the Online Change of Address form, you may complete the Change of Address form below and upload or deliver to your child(ren)'s school(s) along with your proofs of residency and judge signed custody documents if applicable for processing.

Change of Custody and/or Guardianship Information(Currently Enrolled Students Only)
If parents are divorced, please upload the following documents.- Judge signed custody documents showing legal custody, primary physical custody, final decision on education, and the visitation schedule. This information is usually found in your Final Decree and/or Parenting Plan. Child support documents cannot be accepted.
- If the parent with primary physical custody has moved, please complete the Online Change of Address form and upload your proofs of residency.
- A Registration Information Form and picture identification may also be required if not already on file.
If parents are separated and there are no judge signed custody documents, please upload the following documents.- Completed Forsyth County Schools Adult Affidavit (form is available at your child's school).
- A Registration Information form and picture identification may also be required if not already on file.
- If the parent that is primary for school purposes has moved, please complete the Online Change of Address form and supply your proofs of residency.
If guardianship has been granted to a Grandparent, please upload the following documents.- Completed Georgia Power of Attorney for the Care of a Minor Child by the parent(s).
- Completed Registration Information Form by the Grandparent(s).
- Proofs of Residency from Grandparents(s).
- Picture identification of the Grandparent(s);
- Must be valid; not expired driver's license, state id card, passport, or military id.
If guardianship has been granted to someone other than a Parent or Grandparent, please upload the following documents.- Judge signed Temporary Guardianship or Division of Family and Children's Services Placement Letter.
- If you have applied for Temporary Guardianship but have not received the final copy, you may provide the receipt from Probate Court and complete a Forsyth County Schools Adult Affidavit (affidavit is available at your child’s school).
- The affidavit will be in effect for one school year only (expires the last day of each school year) and cannot be renewed. Forsyth County Schools policy requires that you seek guardianship for your student to remain enrolled in our school district. Please contact the Forsyth County Probate Court at 770-781-2140 to begin the guardianship process. Please upload your judge signed guardianship documents for processing when you receive them.
- If you have applied for Temporary Guardianship but have not received the final copy, you may provide the receipt from Probate Court and complete a Forsyth County Schools Adult Affidavit (affidavit is available at your child’s school).
- Completed Registration Information Form by new guardian(s).
- Proofs of Residency. from new guardian(s).
- Picture identification from the new guardian(s)
- Must be valid; not expired driver's license, state id card, passport, or military id.
If there are no judge signed guardianship documents, please upload the following.- Receipt from Probate Court showing that the Temporary Guardianship process has been started.
- Please contact the Forsyth County Probate Court at 770-781-2140 to begin the guardianship process. Please provide the judge signed guardianship documents to your child's school or the Information Systems Department for processing when you receive them.
- Completed Adult Affidavit (form is available at your child's school).
- Completed Registration Information Form by new guardian(s).
- Proofs of Residency. from new guardian(s)
- Picture identification from the new guardian(s).
- Must be valid; not expired driver's license, state id card, passport, or military id.
If you have a Temporary Protective Order (TPO) that has expired, please upload the following.- Judge signed documents showing that the TPO has been extended or dismissed.
- If the TPO has been dismissed and parents do NOT reside in the same residence together with the child, please provide judge signed custody docs showing legal custody, primary physical custody, final decision on education, and the visitation schedule. This information is usually found in your Final Decree and/or Parenting Plan. Child support documents cannot be accepted.
- If there are no judge signed custody documents at this time a Forsyth County Schools Adult Affidavit may be completed for each child (the affidavit is available at your child's school).
- If the TPO has been dismissed and parents do NOT reside in the same residence together with the child, please provide judge signed custody docs showing legal custody, primary physical custody, final decision on education, and the visitation schedule. This information is usually found in your Final Decree and/or Parenting Plan. Child support documents cannot be accepted.
