Dear Parent(s) and/or Guardian(s),

The following survey will be used to indicate which educational setting you are choosing for the 2021-2022 school year.  One survey should be submitted for each student indicating whether he or she will attend school in-person or through virtual instruction.

Selecting the in-person educational setting at the end of section 1 of the survey will submit your response.

Selecting the virtual learning educational setting will open an additional section of the form with a link to the Virtual Learning Participation Agreement.

The selection of in-person or virtual learning for the educational setting is at a minimum a semester-long commitment.

Families will have the opportunity to indicate their choice of educational setting for the first semester during the survey window of Monday, April 12th at 8:00 am through Friday, April 30th at 4:00 pm.
