If your student has asthma and uses an inhaler, the Clinic needs an Asthma Action Plan on file. The form is available in the Clinic or can be downloaded from the Clinic webpage. Inhalers will be stored in the Clinic unless the physician has demonstrated the proper use and requests that your student carry theirs. Inhalers should stay with the student at all times during the day. Please make sure that the student’s name is clearly written on the casing.
If your student has a food allergy, the Clinic needs a Allergy Action Plan on file. The Cafeteria will not recognize the food allergy without the proper form. The Food Allergy Action Plan is available in the Clinic or can be downloaded from the Clinic’s webpage. The physician must sign this form, good for one year. Your child’s liquid Benedryl and Epi-pens will be stored in the Clinic unless the physician requests that your student carry theirs. Please make sure medications are clearly labeled.
If your student has an insect allergy, the Clinic needs an Allergy Action Plan on file. The form is available in the Clinic or can be downloaded from the Clinic' webpage.
