How do I get an ADAP form?

How do I get a Work Permit?
Effective July 1, 2015, with the exception of minors in entertainment, work permits are no longer required for youth age 16 and older.
1. Student completes Section A of Work Permit Data Sheet.
2. Student prints Work Permit Data Sheet and takes it to their new employer to complete Section B.
3. Student brings completed Work Permit Data Sheet, birth certificate, and valid drivers' license to the main office.

How do I get a Certificate of School Enrollment?
Students are required to have a Certificate of School Enrollment completed by the school before they can get a learner's permit or a driver's license. These forms are not available in electronic copy. Student must go to the Main Office and complete "Part A: Student Information" of the form while they are there.
The Main Office will then verify information and complete the school's portion of the certificate. The Certificate of Enrollment should be ready to be picked up within 48 hours. Certificates of Enrollment cannot be faxed or emailed.
