After submitting the interest form.....

Please click the NFMS Esports permission and registrations slip to view the permissions form. There is a $15 fee to help play club dues and cover costs of games, equipment, etc. This will be put in your child's mypaymentsplus account.

This form will include all links to privacy rights for students and parents to review at their will and includes CRUCIAL information for how the E-Sports club will operate. Return this form to Coach Day (room 451) signed by both student and parent. Please return all forms by Thursday, September 15th. No forms will be accepted after that date so that the club can move forward with planning.
