• Current Allergy Action Plan - signed by the doctor
• Parent Questionaire complete
• Medication Form for each medication being left in the clinic
• Request to Carry Form if intending to carry med - signed by the doctor
• Epipen/Auvi-Q - not expired 
• Benadryl - not expired
