    6th and 8th grade parents/guardians,

     

    With every new school year, the state mandates specific health screenings for middle school students, which occur during the school day at school.

     

    This year, 6th and 8th grade students (only) will participate in scoliosis screening.

     

    Many children may have already been screened in their yearly physical or are already under doctor's care for the treatment of scoliosis. If this is the case for your child and you do not wish to have your child screened at school, it is imperative that you sign and return a refusal form to your school nurse. The only way for your child to be excused from scoliosis screening is by providing a refusal form to the school. Below you will find information on scoliosis screenings, and links/directions on how to access the two different options for refusal forms.

     

    Option #1 Complete Electronic Refusal Form: The online scoliosis refusal is found on Parent Portal: Click on the Documents section in the margin, and then complete the Scoliosis Refusal Form by clicking on that option. See example below:
