Georgia State Law requires all 7th Graders must have a Meningitis (MCV4) and a Tetanus (Tdap) Vaccine on a GA3231 Immunization Record before entering the 7th Grade year.
Please check with the school nurse or your physician if you are not sure if your child’s immunizations are current.
If your student's immunization record (GA 3231) is not updated by the first day of school, they will NOT be able to start school.
When a vaccine is administered, please have the doctor review the updated record (GA 3231). If any errors are found by Forsyth County Schools the record will NOT BE ACCEPTED.
