End of Year Clinic Reminders
I have enjoyed helping to care for the health needs of your children this year! There are many things to think about as we approach the last week of school. Here are just a few reminders from the clinic:
End of School Year Medication Pick up – Please pick up ALL medications from the school clinic by 1:30 pm on Thursday, May 25th. Please come to the front office and give your student’s name to the Receptionist. The nurse will bring the meds out to you along with a form to sign. If medication is not picked up from the clinic by 1:30 pm on May 25th, it will be considered abandoned. Abandoned medication will be destroyed in accordance with local, state, and federal laws. We do NOT keep any meds in the clinic over the summer.
New Health Forms/Meds for the 2023-24 School Year - With each new school year, it is necessary to update your child’s Health Forms in the school clinic. All students with medical conditions (ie Asthma, Allergies, Seizures, etc.) must submit new health care plans each school year. The forms must be completed, signed by parent and physician, dated after 5/29/23, and turned in to the School Nurse when school resumes in August. If you plan to bring medications to the clinic next year, you must complete a Request for Administration of Medication form with each medication turned into the clinic. If the medicine is over the counter, only a parent's signature is required. However, if it is prescription medication, or needs to be administered for more than 2 weeks, the form must also be signed by a physician. Please see the school website: Student Services - School Nurse - Medications - to read all the information regarding safely transporting and keeping meds in the clinic.
If you would like for your student to carry emergency medication, such as an inhaler or epi-pen, the physician must indicate on both the Care Plan and the Permission to Carry form that the student is trained and capable of carrying and self-administering the listed medication. The Permission to Carry form also requires student and parent signatures.

Also, please contact your student’s primary care provider to see if they will need any required immunizations before school resumes in August.
