This form is for families to request a Chromebook for use at home.  The form is unique to each school so only fill out this form for your student(s) attending this school.  If you have students attending several schools, complete those schools' surveys for which students attend. FCS has over 52,000 students and a limited number of Chromebooks.  Priority will be given to those with a financial need. Completing this request form is not a guarantee to receiving a device.

 

You will need your student's legal name, student number and current grade level.  Please gather that information prior to beginning this form.
