End of Year Clinic Reminders

I have enjoyed helping to care for the health needs of your children this year! There are many things to think about as the end of the school year approaches. Here are just a few reminders from the clinic:

End of School Year Medication Pick up – Please pick up ALL medications from the school clinic by 2pm on Thursday, May 22nd. Please come to the front office and give your student’s name to the Receptionist. The nurse will bring the meds out to you along with a form to sign. If medication is not picked up from the clinic by 2pm on May 22nd, it will be considered abandoned. Abandoned medication will be destroyed in accordance with local, state, and federal laws. We do NOT keep any meds in the clinic over the summer.

New Health Forms/Meds for the 2024-26 School Year - With each new school year, it is necessary to update your child’s Health Forms in the school clinic. All students with medical conditions (ie Asthma, Allergies, Seizures, etc.) must submit new health care plans each school year. The forms must be completed, signed by both parent and physician, dated after 5/23/25, and turned in to the School Nurse when school resumes in August. • Epipen and/or Benadryl require an Allergy Action Plan
• Asthma medications require an Asthma Action Plan
• Seizure Medications require a Seizure Action Plan
• Insulin and diabetic supplies require a Diabetes Management Plan
• Use the Individual Healthcare Plan for conditions not listed
• For any medications to be kept in the clinic a Request for Administration of Medication Form must be turned in with the medication

If you plan to bring medications to the clinic next year, you must complete a Request for Administration of Medication form with each medication turned into the clinic. If the medicine is over the counter, only a parent's signature is required. However, if it is prescription medication, or needs to be administered for more than 2 weeks, the form must also be signed by a physician. If you would like for your student to carry emergency medication, such as a prescription inhaler, epinephrine auto-injector, or insulin/diabetic supplies, the physician must indicate on both the Care Plan and the Authorization for Students to Carry form that the student is trained and capable of carrying and self-administering the listed medication. The Authorization for Students to Carry form requires physician, student, and parent signatures.

If you have a rising 7th grader, please contact your student’s primary care provider to see if they will need any required immunizations before school resumes in August. ALL students entering the 7^th grade are required to have received both the Tdap Booster (Tetanus, Diphtheria, Pertussis) and the Meningococcal Vaccine (MCV4) prior to the first day of their 7^th grade school year. An updated 3231 form with these newly given immunizations documented must be on file with us prior to August 1^st, 2025.
