Forsyth County Schools reimburses School District employees for reasonable and necessary expenditures incurred in connection with approved travel on its behalf. The School District follows the Statewide Travel Policy, as established by the State of Georgia. School district employees requesting reimbursement for their approved travel expenses may find all pertinent information in the Employee Access Portal.

Due to the recently passed Paperwork Reduction Act, 1095-C Forms will no longer be mailed. Full-time eligible employees, and  non-full-time employees or non-employees currently enrolled in or previously enrolled in the plan, have the ability to request a copy of your 1095-C form by emailing vanessa.stephens@alliant.com, contacting 877-201-0487 or by mailing your request to 14439 NW Military Hwy. Suite 108-PMB #626 Shavano Park, TX 78231. A copy of your form must be provided to you within 30 days of your request. When requesting a copy of your 1095-C form, please indicate if you wish to receive your form via email or mail. If you wish to obtain your form electronically, please email vanessa.stephens@alliant.com indicating that you are giving your consent to receive your 1095-C form electronically.
