The Inactive Student Records Center utilizes Parchment for all inactive records requests.  Please (Click Here) or go to http://www.parchment.com/u/registration/28694843/account to submit your request.  Please enter Forsyth County Schools Alumni as the school you are requesting records from.

• Requests are processed in the order in which they are received within 5 business days (10 business days during peak times - May, June, July, August, December, and January). 
• Former students that are 18 years old or older must submit their request themselves.
• The parent/legal guardian should submit requests for students that are under 18 years of age.

Please Note:

• Birth Certificates are kept on file one year after a student graduates/withdraws, then shredded in accordance with state retention guidelines. Former students  may  

  contact Vital Records or a Probate Court in the state/county where they were born to request a copy of their birth certificate.

• Immunization records are kept on file 

  one year after a student graduates/withdraws, then shredded in accordance with state retention guidelines.  Former students may contact their former physician or Health Department to request their immunization records.  

• Diploma copies are not available.  Former students may request their transcript as proof of graduation.
• ACT and SAT scores must be requested from the testing agency.
• 504 plans are maintained 5 years after the termination of the 504 plan in accordance with state retention guidelines.
• Please contact the Technical College System of Georgia to obtain GED records.
• If you select Other Document as the type of document you are requesting in Parchment, you must email studentinformation@forsyth.K12.ga.us with your legal name, date of birth, and explanation of what document you are requesting after you have submitted your Parchment request.

Records Requests for Inactive Students

• Due to the requirements outlined in Georgia HB 268, a signed (by a school official or parent/legal guardian) records request listing the document(s) you are requesting for students that have withdrawn may be mailed, emailed, or faxed to:

• In accordance with Georgia House Bill 268 records will be sent electronically to your school by 5:00 pm on the third business day after receipt of a signed (by a school official or parent/legal guardian) records request listing the document(s) you are requesting.  These records will be securely sent to you with an eight-digit pass code to access.  You will be contacted with the pass code.

• If you require a paper copy of the records with a seal you will need to specify on your signed records request form that you wish to receive by mail (USPS).
