    Angie Podmolik is our school nurse at Brookwood Elementary. Nurse Angie is here to provide a safe and healthy environment for our Brookwood community.

    Clinic Hours are 7:00 AM -3: 00 PM on operating school days.

    **Even though daily school attendance is encouraged for all students, please do not send your child to school if they are not feeling well in the morning. Children MUST BE free of fever, diarrhea, and vomiting for 24 hours without medication prior to returning to school.**


    Medication Reminders
    1. The clinic does not stock medications. Over the counter medications MUST be brought to the clinic by the parent. A Request for Administration of Medication form must be completed and signed.
    2. Daily medication that will need to be administered during school hours for a period of longer than two weeks MUST have a physician signature on the Request for Administration of Medication All medication MUST be brought to the clinic by the parent.
    3. Students CANNOT have medications with them on school grounds (unless authorized to carry) including cough drops, sunscreen, etc.
    4. Medications:
      1. Must be in original container with student name
      2. Cannot be expired
      3. A Request for Administration of Medication form must be completed and signed
      4. By the parent and physician.
      5. For seizure, allergy, Asthma, and Diabetic medications, additional forms (care plans) are required and must be completed and signed by the physician and parent.
    5. All medication forms and care plans must be dated after June 1st of the current year.
    6. Medications MUST be picked up in the clinic by the parent.
    7. Any request for an adult dose of medication OR a medication that contains Aspirin, MUST BE SIGNED BY A PHYSICIAN AND PARENT."
