• Welcome to the Hendricks Middle School Clinic home page!

    The clinic is here to keep students healthy so that they can learn. As students transition to middle school, our goal is to promote independence. This can mean helping a student learn how to manage their chronic condition, being responsible for consistently coming to the clinic for daily medication or simply learning how to discern between symptoms that require them to leave school vs being able to manage throughout the school day. The nurse is here to work with students, parents and staff to best serve the individual health care needs of each student within county policy. The clinic is able to distribute medication, evaluate symptoms, and respond to emergencies, in addition to promoting health through education of students and staff. Additionally, middle school clinics must implement the compliance of state required 7th grade immunizations, 7th grade vision screenings and scoliosis screenings for 6th and 8th graders. The ability for the clinic to manage the needs of the Hendricks community comes from county funding, Northside grants and the generous contributions from our PTO.

    In the event the school needs to reach you:

    • Please make sure the Parent Portal has the correct contact information. Update your phone number and email. 
    • Consider saving the school's phone number (678) 965-5038 in your Contacts list so you see us on caller ID.  How about a dedicated ring tone? 
    • List all adults (18 and older) able to pick up your student and designate Emergency Priority options in Parent Portal.
      • Consider having a responsible adult/friend/neighbor listed in Parent Portal on standby in the event you are unable to get to the school to pick up your ill child in a timely manner.
    • Photo ID is required to pick up any student.

    For students with chronic/long-term conditions or medication needs, please have the appropriate forms (see link) filled out carefully and legibly and signed by the pediatrician or specialist.  This information will be used to care for your child while at school or on a field trip. These care plans are good for one calendar year.
