• If the nurse deems the student to be ill or injured, the nurse will contact the parent to pick up their child from school.  If the parent states that they want their child to drive home, the parent must fax or email  the statement below to the school nurse.
     
     
    Parent Permission for Driving Home
     
    The school nurse has made me aware that my child, _____________________, is ill or injured and has recommended, as well as asked, that I pick up my child from school.  I am directing the school to allow my child to drive home and will not hold the school system or its employees liable for my decision.
