Applications for Forsyth Academy can be downloaded from the Academies of Creative Education website. Click here to access the page for the Forsyth Academy application.  Students and parents should complete the student section, then take the application forms to their base school counselor. The counselor will complete the remaining sections and send to the Data Clerk at Forsyth Academy. After processing the application, an administrator from Forsyth Academy will contact the student and parent for a conference and orientation meeting.
