Rising 11th Grade Students-Meningitis Booster Dose School Clinic
- Meningitis booster dose (MCV4) is a GA state requirement for rising 11th-grade students who are 16 years of age. Please see attached parent letter below for more details
- Department of Public Health and FCS are partnering to offer this immunization to current eligible Lambert students on March 8th during your child’s school day.
- Your child must be 16 years of age by March 8th to be eligible to receive this MCV4 booster dose at this event.
- Students must have a completed permission form (all 3 sections must be completed) and signed by parent/guardian. All permission forms must be turned in to the school nurse by Friday, March 4th.
- All insurance is accepted. If your child has no insurance, then cost is $21 that will be paid directly to the Forsyth County Health Department at the time of the event. If there is a financial concern, please contact your school nurse.
